Survey gauges local health needs

DeKALB – KishHealth System and the DeKalb County Health Department are sponsoring an online survey to assess health needs in the community. Results will be combined with existing federal, state and local health data to provide guidance to identify health problems and fulfill both federal and state mandates.

Participation is voluntary and the online survey is designed to measure residents’ perceptions of health care delivery, access and health practices. The confidential online survey uses standardized questions, is available in English and Spanish and will take approximately 15-20 minutes to complete.

The community health needs assessment process ensures the Kishwaukee Community Hospital, Valley West Community Hospital and DeKalb County Health Department are responsive to their communities by acting to address priority health needs.
